Where a nurse or midwife wishes to practise in a different name, she/he should provide: * a marriage certificate issued by a government authority (such authorities are frequently titled Registrar of Births, Deaths and Marriages or similar); ceremonial certificates signed by a clergy or other celebrant are NOT adequate. * other legal document issued by a government authority confirming or authorising a change of name ( such as a deed poll registered by the Registrar of Births, Deaths and Marriages or equivalent government organisation). * change of name entered on an amended birth certificate or passport issued by the appropriate government authority.
All photocopies of documents submitted must be certified by a Justice of the Peace or Legal Practitioner. |
