Application for Medication Endorsement for enrolled nurses who have undertaken
appropriate addditional education and seeking endorsement to administer medications
Annual Return Declaration Form for nurses and midwives who have not otherwise
completed a return (usually provided as part of annual renewal notice)
Certification of Employment to be used by Registered Nurses or Registered Midwives
who are also Enrolled Nurses who wish to be endorsed for medication administration
